Integrated Care Communities (ICCs) are integrated teams of health and care workers, practicing population health with a mobilised population. Based on natural communities of between 10,000 and 70,000 people, ICCs bring together primary, community and social care workers into one single integrated team working to a common purpose: improving the health and well-being of the local population and are key component in our model for delivering care closer to home.

 

Comprised of health and social care professionals, third sector organisations, and members of the local population, ICCs work collaboratively using a Population Health frame of reference.

 

There are 11 Integrated Care Communities in Morecambe Bay reflecting the geography and natural communities in existence.

 

The Kendal ICC team are looking to recruit a Case Manager – ICC Care Home Project to lead specialist in a defined area of nursing care.

 

You will assess, plan, implement and evaluate programmes of evidence-based nursing care. Provide expert, specialist advice to patients/service users/residents, carers/relatives, clinical practitioners and peers. Provide specialist education and training to others. Work collaboratively with clinical colleagues to develop services and quality of care delivered. Perform highly specialist nursing procedures. Lead the development of clinical services within the specialist field. Undertake research and undertake audit in own specialist area. Work with other ICC staff to improve levels of care and reduce unnecessary admissions.

 

Anyone newly appointed to the trust will be subject to the completion of a satisfactory 6 month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.
