Hospital Home Care Co-ordinator

The service support patients to return home using the trusts supported discharge model and to continue their rehabilitation pathway in the community. The service is supported by a team of Physiotherapists and Occupational Therapists and Clinical Therapy Support Workers, thus enabling the patient’ rehabilitation journey to continue in the community in their own homes.

 

The service also provides high quality outreach care at home for our patients whilst they are awaiting their permanent care provider to begin. It provides support to the patients with their activities of daily living thus enabling them to remain in their own home. The service also supports patients in the terminal phase of their illness providing end of life care needs and psychological and spiritual support to patients and their families.

 

The service requires an experienced co-ordinator to support the day to day supervision and co-ordination of the service, and provide a troubleshooting supportive role to the Clinical Therapy Support Workers in the community.

 

You will be required to develop strong links with other members of the multi-disciplinary team members involved in the discharge process and the on-going care needs of the individual patients in the community

 

You will be expected to use your own initiative and be able to work unsupervised, undertaking specific work including audit as necessary, as this role is pivotal to driving this service forward.

 

You should have a good working knowledge of e-roster / e-Pay and Microsoft Office including ‘Word’ and Excel as well as the ability to effectively communicate and establish links with other professionals.

 

In addition to the office workload the co-ordinator will be required to support the staff and patients in the community in terms of staff supervision and patient care when service demands are needed (i.e. to cover staff sickness) This will involve supporting patients with an exercise programme and therapy treatments and assisting with personal care and activities of daily living

 

You will be required to travel to patients homes within the trusts geographical area and within rural locations – please note having your own transport/the ability to travel is an essential requirement to this role and you will be required to have business on your vehicle insurance.

 

Please also make it clear on your application that you hold a UK driving license and are prepared to take out business insurance.

Anyone newly appointed to this trust will be subject to the completion of a satisfactory 6 month probation period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.

 

Please note that these posts will cover patient visits within Lancashire, Furness and the South Lakes area whilst based at Furness General Hospital

 

Hours of service operation are from 08:00-21:00 7 days a week

 

Contracted hours = 36 hours per week

 

Shift pattern will be a combination of 08.00 – 21.00 x 3 days (36 hrs) and/or 08.00 – 21.00 x 2 days and 09.00 -15.00 x 2 days (36 hrs)

 

This post is fixed term until 31.10.19 to cover maternity leave.
