Supported Discharge Team – Specialist Physiotherapist Band 6


We are recruiting a specialist Physiotherapist to provide therapy-based assessment and treatment to a new Supported Discharge team to be based at Westmorland General Hospital, Kendal, Cumbria.


The team will deliver a Discharge to Assess and rehabilitation service. This is a new service to residents within the Morecambe Bay and South Lakes locality. The Team will be based at the Westmorland General Hospital, Kendal, Cumbria and provide assessment and rehabilitation for patients on discharge within the person’s home rather than in hospital as part of a Discharge to Assess and Supported Discharge service.


You will join a team of Occupational Therapists, Physiotherapist, Rehabilitation technical instructors to assess, treat and also to support and identify discharge requirements, making referrals on to Health and Social care community services.


All assessment will be within the patient’s home at point of discharge from hospital


The Therapy Team will deliver a 6 day service over an extended day of 08:00-16:00.


You will have broad previous experience within an acute inpatient, admission avoidance or community-based rehabilitation services.


You will have knowledge and experience in the assessment, treatment and goal planning with patients presenting with varied physical conditions and will be motivated to join us in improving our patients’ experiences and outcomes.




A Diploma or Degree in Physiotherapy approved by the Chartered Society of Physiotherapy.


Registration with HCPC as a Physiotherapist


The ability to travel to a variety of venues throughout the community to deliver patient assessments within an identified locality and to attend meetings


In addition to a comprehensive induction package we offer Team support, staff supervision and in-service training opportunities


Anyone newly appointed to the trust will be subject to the completion of a satisfactory 6 month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.
