An opportunity has arisen within the Rapid Response Team North for a band 6 senior community nurse. You will be an innovative enthusiastic nurse, ideally with a background in nursing medical patients; to work within our community based Rapid Response and IV Therapy Team across the Lancaster & Morecambe Localities.

 

The service supports the Trust’s Unscheduled Care Agenda providing an alternative pathway that will treat people at home in order to reduce unnecessary hospital admissions and facilitate early safe discharge.

 

You will be able to demonstrate effective leadership, team and partnership working and understand the context and vision of health and social care.

 

This is an opportunity to work in a team that will collaborate with both Primary and Secondary Care Providers to provide suitable patients with Nursing Care / rehabilitation at home over a limited period of time, pending initiation of longer term support from established community Health and Social Services.

 

This post requires a proactive individual with excellent clinical knowledge and communication skills have evidence of ongoing professional development and be able to work independently. A flexible, professional approach is essential to the role. A car driver with use of a vehicle is also essential. The service operates 7 days a week

 

Anyone newly appointed to the trust will be subject to the completion of a satisfactory 6 month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment.
